An essential step in the immigration process and a must for filing a green card is the Immigration Medical Examination. This process is crucial, wherein the applicant for a green card must seek a civil surgeon who is a licensed physician authorized by the US government to conduct the immigration medical examination. GoodLife Medical Center provides an immigration medical examination for individuals obtaining green card status with the United States Citizenship and Immigration Services, or USCIS. First Appointment
Preparing your documents ahead of time will help you to have a fast transaction when you visit the office. Here is the checklist for your first appointment that GoodLife Medical Center will collect from you:

Check the immunization record for any missing immunizations
GoodLife Medical Center will give you a laboratory order for:

Tuberculosis

Gonorrhea
-
Syphilis

You will be issued a blood work order and directed to the lab at this address: AEL Lab, 2120 Exeter Rd., #110, Germantown, TN 38138.
Lastly, you'll make a payment for the immigration physical using a credit card, cash, money order, or cashier check. The cost is $375 with insurance or $530 if you do not have insurance. The additional cost for non-insured patients covers the fees for the lab work done.

Second Appointment and Medical Examination
Your second appointment will happen five to seven business days after your initial visit. This will give you ample time to complete your paperwork. Once the blood test results are normal and the vaccination requirements have been met, GoodLife Medical Center will contact you for your scheduled appointment to see the Civil Surgeon. If there are unusual results, we will contact you for the next steps before seeing the Civil Surgeon. If you have an interview or your I-693 form must be submitted to immigration by a specific date, please let us know.

Medical Examination
The GoodLife Medical Center’s civil surgeon will conduct a thorough medical examination. The Civil Surgeon will conduct an accurate medical history, physical, and mental evaluation, as well as review laboratory results such as drug tests, alcohol screening, and test results for various diseases and illnesses.
After the medical examination, GoodLife Medical Center will complete the I-693 form signed by the Civil Surgeon and yours. Once it is completed and we've ensured that all necessary information in the I-693 form is filled out, the Civil Surgeon will seal the envelope that you will send to Immigration. In addition to this, we will give you a copy of I-693 for your records.
